Manual booking form

Use the manual form if you experience any problems with the online form. To open the form click download, then go to your downloads folder and open the file to enable editing.

The form is provided as an MS-Word file which can be filled out electronically and a PDF file which can be printed and filled out by hand and then scanned.

Complete the form version you prefer and email it along with a copy of the vessel’s Certificate of Currency of your insurance to slipway@mmyc.com.au
